Closing Statement
This report provides option, restricted stock, and PIU Profit Interest Unit - a unit that confers an interest in the profits of the company as determined by the board of directors for Limited Liability Companies (LLC) information for the selected participant An individual or entity participating in an equity compensation plan and/or employee stock purchase plan whether terminated already or to be terminated in the future.
Report Parameters and Filters
Report parameters and filters are used to tailor a report to suit your requirements. When you have selected all of the criteria needed to run the report, you can save it for future use by scrolling to the Save Report Parameters section, entering a unique criteria name, and clicking Save Report Parameters.

- Report Type – select Web Report (HTML), Excel File (CSV), Tab Delimited (TXT), Excel File (XLSX), PDF File (PDF), Excel File - Detailed (XLSX), or Customize Report (only displays if the Customizable column = Yes)
- Report Date Range – enter a report end date that is the participant's future termination date (start date is not considered) or leave blank to use the termination date of a participant that is already terminated
- Participant Name – click the icon next to the participant name field, enter part or all of a name or participant ID in the search field (or multiple complete IDs separated by commas or semicolons), click the search icon, check all matches that apply, and click OK to close the window or Cancel to close the window without saving the filter matches.
Note: You can continue to build a list of participants using this filter. After checking the matches that apply from the first search, do not close the filter window. Clear out the search criteria in the search field and enter new search criteria to locate your next set of matches, click the search icon, and check the matches that apply. Repeat until you have completed your search. You can have up to 500 results from your searches. - Termination Type – select Cause, Voluntary, Involuntary, Death, Disability, or Retirement from the dropdown list
Note that if you do not select a termination type, the system uses "Cause" for active participants. - Run as Summary Only – check the box to run the report as a summary only; for example, multiple rows with data for one participant are summarized into one row
